Please read these conditions carefully before using the Sussex Imaging Partnership LLP website.  By using this website, You signify Your agreement to be bound by these conditions.

Definitions

Healthcare professionals or visitors to this web site are referred to as “You” or “Your”

Sussex Imaging Partnership LLP are referred to as “Us” or “We”.  Sussex Imaging Partnership is the trading name of Sussex Imaging Partnership LLP.

1.  Referral Confirmation

When You send an electronic E-Referral Request Form to us We will send You an e-mail confirming receipt of Your E-Referral Request Form.  We will then contact our sites who will contact your patient and arrange the date and time of the examination.  Your submission of a E-Referral Request Form represents a request to Us to carry out an imaging examination or procedure which is accepted by Us when We arrange a suitable appointment with the patient.

If You have incorrectly entered Your email address Your confirmation and dispatch email may fail.  Failure by Us to send a receipt confirmation email in no way invalidates Your Referral Request or Our acceptance of your Referral Request.

If you fax a Referral Request Form to us, we will contact our sites who will contact your patient and arrange the date and time of the examination. Your submission of a Referral Request Form represents a request to Us to carry out an imaging examination or procedure which is accepted by Us when We arrange a suitable appointment with the patient.

2.  Post 

We send Reports by Royal Mail.

FULL VALID POST CODES are required for every Referral Request order.   If You provide an incorrect address, including the postcode, We do not accept any responsibility for a failure of a Report to arrive or any costs involved in redirecting that Report. 

We strive to dispatch Reports within 24 hours of the examination, although this is not always possible, and depends upon the nature of the examination and the requirement to review with colleagues.  If Reports sent by Royal Mail are not delivered please contact Us.

4.  Prices 

All prices quoted are in £ Sterling.

We endeavour to ensure that the information shown on the web site is as accurate as possible however we cannot guarantee that information and offer no warranties as to the accuracy.

We cannot be held responsible for typographical errors on this site, including but not limited to those regarding price. We reserve the right to make changes to Our web site including prices, procedures and terms & conditions without any notice being given.

All services are subject to availability and may be withdrawn at any time.

If You register to join our database in order to send E-Referral Requests Forms as a Healthcare Professional , You are responsible for maintaining the confidentiality of Your unique URL and for restricting access to Your computer to prevent unauthorised access to Your unique URL.

You agree to accept responsibility for all activities that occur under Your unique URL. You should take all necessary steps to ensure that the URL is kept confidential and secure and should inform Us immediately if You have any reason to believe that Your URL is being, or is likely to be, used in an unauthorised manner.

7.  Access to Our Website

We will do Our utmost to ensure that availability of the website will be uninterrupted and that transmissions will be error-free.  However, due to the nature of the Internet, this cannot be guaranteed.

Your access to the website may also be occasionally suspended or restricted to allow for repairs, maintenance, or the introduction of new facilities or services.  We will attempt to limit the frequency and duration of any such suspension or restriction.
